The following form is to be used to document technical assistance given to Wyoming National Board Certification Candidates by trained candidate support providers. To be eligible for payment you will need to fill in the information completely. Please be specific with regard to the Type of Acvtivity. Please indicate if the time logged was spent in your cohort. For monitoring purposes, updates must be submitted every two weeks. Final documentation recorded no later than June 1 each year.
