Candidate Support Provider/Candidate Agreement

20___ - 20___This agreement is designed to help Candidate Support Providers (CSPs) and National Board Candidates establish a relationship that will enhance the professional growth experience of seeking national board certification. The relationship between a Candidate and a Candidate Support Provider is based on trust, respect and a clear understanding of the role each party fulfills. This relationship is more successful when all parties agree upon what is expected. Please discuss the statements below and initial the items when both parties have a clear understanding of and agree to the item.

The Washington Initiative for National Board Teacher Certification provided this model agreement for the use of the Wyoming National Board Certification Initiative. We are thankful for their assistance in building our support system.
